Older people’s and informal caregivers’ empowerment in transitional care decision-making: Policy and practice
The problem Older people (and their informal caregivers) are more vulnerable when moving from one care setting to the next (1). They are often confronted with poor communication and management of these transitions, resulting in a lack of knowledge and difficulties in making transition decisions, ultimately leading to mental burden, medication errors, missed diagnoses, incorrect treatments, poor health outcomes, and inappropriate placement in long-term care facilities (2-9).
What can be done?
To optimise (and avoid unnecessary) care transitions, it is vital to understand how we can empower older people and informal caregivers in the transitional care decision-making process.
